Booking an Appointment

There are 3 very easy steps to booking an appointment with us:

 

Step 1: Submit or update your patient record.

This is required for both current and new patients prior to scheduling your Annual Eye Exam.

For your convenience, there are two options for completing this form.  Choose the most convenient one for you!

  • Electronic Patient Form: (preferred) This option can be completed conveniently online and is automatically submitted to the practice. Please note, we recommend using Google Chrome to complete the online forms.
  • Downloadable, printable Patient Form:  This form must be printed and mailed to our office.  It must be received by us before we can confirm your appointment.

Step 2: Request your appointment.

Now that your forms are submitted, you can request a convenient time for your appointment.

Step 3: Appointment Confirmation.

Once we’ve received both your appointment request and patient form, we’ll reach out to confirm your appointment.
